* Do you work? Describe your job and whether you are full time/part time or casual.
*Do you have a drivers licence and car in order to transport a foster dog and take it on outings? This is required for AWDRI foster carers. If you do not have a car or licence please explain how you will transport the dog on outings or to the vet.
* Do you currently own, or have you previously owned, a working type breed? eg Australian Cattle Dog/Heeler, Stumpy Tail Cattle Dog, Border Collie, Kelpie or Koolie
* Do you currently have any pets? If so, what kind? Breed, age, gender, desexed? If you have an entire pet (not desexed) please explain why.
* If you said yes to the above, has your current dog been C5 vaccinated in the last 12 months? (C5 includes kennel cough)
* If you have a dog currently where does it sleep? Inside or Outside? Describe its sleeping arrangements.
* Where will your foster dog sleep? Describe the sleeping arrangements you have for the dog.
* Please describe your understanding of the exercise needs of a working type dog?
* Are you able to meet the needs above while the dog is in your care? Describe what type and amount of exercise you plan to participate in with the dog while it's in your care.
* Do you prefer to foster pure breds or are you happy to accept cross breeds also?
* Is your yard securely fenced and without gaps or holes a dog could fit through? Please describe type (materials) and height of fencing. Please note that when applying to foster for AWDRI a home check will be performed wherever possible to confirm this.
* Who will be the main caretaker of this dog while it is in your care?
* If there is no-one at home where will the dog stay? Inside or Outside? If outside please describe where the dog will be contained including type of shelter.
* Will the dog be allowed inside? For how long each day?
* How many hours will the dog be left alone for each day?
* What will you do with the dog in your care if you have to go out of town or move?
* Do you have a swimming pool? Is it fenced off so that the foster dog cannot get to it?
* Do you have any children or other adults living with you? Please list them, their ages and sex.
* Are you renting or do you own your home? If you are renting do you have the owner's/landlord's permission to care for a foster dog?
Do you currently have a vet? If so please provide their name, phone number and clinic address so that we can contact them for a reference.
