Please complete this form if you are seeking our help in rehoming your dog. Please note that completing this form does not guarantee that Australian Working Dog Rescue Inc will be able to find a home for your dog and you should exhaust every channel at your disposal to find your dog a suitable home.

AWDRIwill accept adoption requests and enquiries in regard to the listing for your dog and will pass these through to you, the owner, but AWDRI bears no responsibility for the suitability of any applicant.  You, the owner, must perform your own checks as to the veracity of the information provided to AWDRI by the applicant.

Be aware that should AWDRI be able to take your dog for rehoming that you will be required to sign a Change of Owner form to relinquish ownership of the dog.

We recommend that all dog owners seeking to rehome their dog read the page on Rehoming on the PetRescue website for additional suggestions on avenues to use in advertising your dog and also for possible ideas on how you can keep your dog.

Is the dog vaccinated? If so, level of cover (eg C3, C5 etc)
Is the dog currently on intestinal worming treatment?
Is the dog on heartworm prevention? If so, is it monthly or annual?
Please insert any link to photo(s) and video(s) of dog (eg PhotoBucket or FlickR etc). Please note that dogs that have no photos or poor quality ones are nearly impossible to re-home. At minimum two photos will be required for a good result.
